Personally Identifiable Information. You can visit the Site without sharing personally identifiable information, but you will be required to provide personally identifiable information in order to use certain Site features, such as creating a Portal account and populating your Portal profile. Specifically, we collect your personally identifiable information in receiving and fulfilling information requests, setting up your Portal account and profile, and when you respond to communications from us (e.g., surveys and invitations to provide feedback). If you provide your wireless or other telephone number to us, you consent to receiving telephone calls and text (SMS) messages from us at that telephone number as described below.

If you are a Portal navigator, information submitted as part of your profile may also include: your applicable training and educational background, a narrative about yourself to be shared with Portal users, background information about yourself regarding why you want to be a Portal navigator, your availability, your preferred methods of contact, other information, and a photo of yourself. If you provide your telephone number, you consent to receiving telephone calls from ACMF and its designees at that number in connection with your role as a Portal navigator. You may also request ACMF to post your telephone number and email address on your Portal navigator profile so that other users of the Portal can contact you. Note that the Portal is publicly available and therefore any person who visits the Portal will have access to your telephone number and email address; ACMF has no control over how Portal visitors use your telephone number and email address and ACMF is not responsible for that use.

Email Portal Content to a Friend. If you choose to use the Portal’s feature that generates an email to a friend or other person that points to particular Portal content, you will need to provide us with that person’s email address. We will use that email address solely for purposes of sending that email.
